Ordering Instructions:

• Vendor submits proposal to customer using KCDA pricing.
• Proposal must reference KCDA Bid #09-005.
• Customer issues purchase order to the vendor c/o of KCDA. The purchase order must not be made out to KCDA.
• The address on the purchase order should reference the vendor’s address.
• The customer’s purchase order must reference KCDA Bid #09-005. Clarification for ordering can be received by contacting the John Endicott at 425-251-8115, x 129.
• KCDA will verify the pricing (which includes the 2% KCDA service fee) is what was submitted by the vendor in the vendor’s bid response.  If it is not, KCDA will contact the customer or the vendor.
• If there are no questions regarding the pricing, KCDA will e-mail the vendor and customer confirming receipt of the customer purchase order and that we have verified pricing is accurate and agree the order can be processed by the vendor.
