You may request a return authorization by fax or electronically by filling out one of the posted forms.

 

1. Please fill the form out using information found on your invoice or packing slip.

2. Fax or submit electronically the request to KCDA.

3. KCDA will contact you with an approved RMA number and instructions for returning the item(s). 

4. Please do not return item(s) until you have been contacted by KCDA as this may delay your credit.
